Osteoarthritis Assessment and Management

Osteoarthritis is a whole-joint condition that can cause pain, stiffness and reduced function. Symptoms do not always match the severity of changes seen on imaging, so management should be based on the individual rather than an X-ray result alone.

Assessment Comes First

Assessment considers the affected joint, symptom pattern, function, examination findings, previous treatment, imaging, general health and personal goals. Other causes of pain may need to be considered before symptoms are attributed to osteoarthritis.

Core Management Options

Management commonly includes education, appropriate physical activity, progressive strengthening, load management and support for relevant health factors. Footwear or orthotic strategies may contribute to selected lower-limb presentations.

Weight Management Without Stigma

For some people with knee or hip osteoarthritis, reducing excess body weight may improve symptoms and general health. Dr Carlos Tahuil-Ochoa — Specialist General Practitioner can discuss an optional evidence-based weight-management pathway when clinically relevant. Weight loss is not a condition of access to assessment or rehabilitation.

Medication and Procedures

Medication review belongs within an individual medical assessment. Selected patients may discuss an image-guided procedure after the diagnosis, alternatives, evidence, likely benefits, limitations and risks have been reviewed. No procedure can guarantee pain relief or cartilage restoration.

When Referral May Be Appropriate

Specialist or surgical referral may be considered when symptoms and functional limitation remain substantial despite appropriate non-operative care, when the diagnosis is uncertain, or when red flags or unexpected findings are present.

Does severe osteoarthritis on X-ray always mean severe pain?

No. Imaging findings and symptoms are related imperfectly. Decisions should consider pain, function, examination and goals.

Appropriately prescribed exercise is generally an important part of care. The type and dose should be adjusted to the individual.
